Qualifying Tests: Dental Work

After four years of good dental health, I found I had a whole lot of new cavities that were probably caused by the two rounds of chemotherapy administered before my transplant.  The work with my dentist took two extended sessions separated by one week to resolve these problems where he did multiple fillings and two crowns.  Since your dental work may be different from mine, I’ve included two extra lines for your PAGs and left the line item component descriptions blank:
