Post-Transplant: Weekly PAGs

I’m including twelve blank PAGs below to record your weekly progress.  You should use them as part of your daily journal to document your post-transplant experience.  Normally transplant patients begin to feel much, much better within two to three months post-transplant, and these should be sufficient to cover that timeline.  If you want to keep a weekly summary PAG after that, add one to your daily journal each week or make copies of this page for your lose-leaf binder journal.


The important thing is to look for the changes – both positive and negative.  You will sometimes see back-slides, but overall you should experience a forward motion.
